Please note that All registration, paperwork and payments must be completed before a player will be eligible to play in any games.
The fees for Spring 2012 High School hockey are $180
Please note that there will be other expenses including a team jersey, additional practice ice and any other team specific fees.
Registration fees are non-refundable once a player is placed on a team. If a player is not placed on a team the registration fee will be refunded.
ONLINE REGISTRATION STEPS
1.) Download and fill out the following paperwork:
2.) Make a photocopy of the following:
· Current High School Identification Card
· Birth Certificate, Drivers License or Passport
3.) All of the above items need to be given to your team manager before the player takes the ice.
4.) Visit www.usahockeyregistration.com to purchase your 2011-2012 USA Hockey Membership. The confirmation code you receive from USA Hockey (9 digits & 5 letters) will be required in the following online registration. Players that played on a USA Hockey team this past fall will be able to use their existing registration.
